We will provide a super bill for you to submit to insurances we are not affiliated with.

 (Contact Kaiser Permanente to see if they will accept an out of network provider.)

Please be sure to verify your benefits prior to your first appointment to inquire about your copay, co-insurance, and/or deductibles that may apply. You may call our office with assistance in verifying benefits for in and out of network services. Most insurance plans cover teletherapy however we strongly encourage you to contact your insurance provider to clarify your specific coverage.

Please note that we have access to the same information from your insurance company that you do.  While we do our best to help you in verifying benefits, it is your responsibility to confirm that your insurance pays for your services and if they don’t we will send you an invoice for any unpaid amount. 

FORMS AND LETTERS

If you need a LETTER or FORM filled out we charge an initial fee of $50 for the first 30 minutes, but will charge extra if the letter or form requires more time than that or needs to be redone.
